Family Clinic's registration process may be completed by printing the forms below. You can then fax or bring them to our office at the time of your visit. This will allow us to verify your insurance in a timely manner. Please be advised that our office will also need a copy of your insurance card, both front and back. If you fax these forms, please include a copy of your card at that time. If you bring the completed registration forms with you, please arrive 20 minutes prior to your visit for our insurance department to verify your benefits.
